Primary care providers increasingly recognize the need to act to reduce health inequities and to reduce the negative health impacts of social determinants of health.  Providers with an interest in social health interventions often do not know who or where their likeminded colleagues are.

The Social Health and Equity Network for Primary Care in Canada seeks to connect health providers interested in addressing health inequities and social determinants of health through the structure and practice of primary care.  This website is intended to facilitate building communities of support, education, program development, research and advocacy.

How to Use This Site

This network is made up of health provider members with an interest in social health and health equity.  More or less specific matchers can be found by completing the search fields above. 

Search results are displayed on the map.  A user profile summary is accessed by clicking on an arrow, and a full profile is accessed by clicking on a member’s name.  

Individual members can be contacted by using the contact form in their profiles.

Primary care providers are invited to join the network by setting up a user profile.
